Camp Cancellation Policy and Important Forms

Cancellation and Change Policy

  • Cancellation by April 1, 2012 = refund minus $25 administrative fee.
  • Cancellation between April 1, 2012 and May 1, 2012 = $100 administrative fee for the first week. In addition there will be a charge of $50 for each additional cancelled week.
  • Changes between April 1, 2012 and May 1, 2012=$50 administrative fee.
  • Cancellation/Changes after May 1, 2012 = no refunds (unless we can fill via a waitlist). Then charge $100 administrative fee.

Medical Forms: You may send the forms to us by Fax, at 617-207-4340, by e-mail to our Medical Records Coordinator, or by regular mail to CRA, PO Box 534, Needham Heights, MA 02494.

Medical Forms Only

Prescription Medications : Authorization to administer prescription medication to the camper while at camp.

Transportation Forms

Transportation : Designates specific persons allowed to transport camper from camp. This is required for car-pools or any drivers other than legal guardians.
