On-line Registration

On-Line registration is handled for Charles River Aquatics by thriva.com. Please read the following information about On-Line registration:
  1. Select the Session (week) that you want and click full day or half day.
  2. You will need to provide emergency contact information, the name of your health care provider and your insurance carrier during the registration process.
  3. While on-line registration is fast and easy, your child’s space is NOT guaranteed until we receive a copy of his/ her physical performed within the past two years. The copy of the physical should arrive at our office within two weeks from the day registered. Since space is limited, if the copy of the physical does not arrive within two weeks, the application will be considered "incomplete" and there is a chance that your child may lose his/her space.
  4. The copy of the physical must include the following: proof of the child’s physical within the past two years that is clearly dated and signed by the physician and specifically grants approval for the child to participate in sports camps; a legible copy of immunization history including a recent Hepatitis B shot. You may use the linked Medical Form.
  5. Make sure you are familiar with our Cancellation Policy and important additional information related to camp activities.

If you already created an account for swim lessons, you can register as a returning user, provided you remember your ID and password.

Mail Registration:

If you do not want to register on-line, you can register by mail. However, please be advised that we cannot guarantee that a session will be open by the time we get your application. It is highly possible that an on-line registration will close a session between the time you mail your application and the time we receive it. It is highly recommended that you list a second choice for a session if you apply by mail.

  1. Please check session availability before proceeeding.
  2. Print the linked Application and Medical forms, fill them out completely, and send them along with a check for $450 for the full day camp ($360 for the 4 day camp), or $325 for the half day camp ($260 for the 4 day camp) to:

    Upon receipt of the application, medical forms (Part A and Part B) and payment, a confirmation email will be sent.
  3. Both parts of the Medical Form must be completed. Part A is to be completed by the parent/guardian. Part B requires proof that the child has had a physical after August 14, 2007. The copy of the physical must include the following: proof of the child’s physical within the past two years that is clearly dated and signed by the physician and specifically grants approval for the child to participate in sports camps; a legible copy of immunization history including a recent Hepatitis B shot.
  4. Incomplete medical forms will be returned and campers may forfeit their space in camp.
  5. Make sure you are familiar with our Cancellation Policy and important additional information related to camp activities.

Note: Checks that are returned for insufficient funds will be subject to a $25 fee.

Additional Information

greenweek2Cancellation and Change Policy

  • Cancellation by April 1, 2010 = refund minus $25 administrative fee.
  • Cancellation between April 1, 2010 and June 1, 2010 = $100 administrative fee for the first week, $50 for additional weeks.
  • Cancellation after June 1, 2010 = no refunds (unless waitlist). Then charge $100 administrative fee.
  • Changes will cost a $25 change fee.

Prescription Medications (12 kb PDF file): Authorization to administer prescription medication to the camper while at camp.

Transportation (13 kb PDF file): Designates specific persons allowed to transport camper from camp. This is required for car-pools or any drivers other than legal guardians.
